Why Document Verification is So Important
Documents verification is done two times during the MP NEET counseling process:
- Once at the time of registration while uploading scanned documents through the online portal.
- And once again when reporting to the allocated college for verifying the documents in person.
The admission committee at the allotted college is authorised to reject your admission if any document is found missing, forged, expired, or incorrect. Rejection at this stage means your allotment is cancelled, you get a written notice, and you must appeal within 2 working days or lose the seat permanently. So treat document preparation as seriously as you treat choice filling.
Documents Required for MP NEET Counselling 2026 – Complete Checklist
The document list below is applicable for MBBS and BDS (Undergraduate) programmes. Documents are required both for portal upload (scanned PDFs under 200 KB) and for physical verification at the college (original + self-attested photocopies).
1. NEET UG 2026 Admit Card / Registration Slip
Take along the Photocopy of your NEET UG 2026 Admit Card/Registration Slip. Your Registration Slip or Admit Card is the key identification document during counselling and should contain the same data that you entered while creating your profile on the website.
Remember: Your Registration Slip should contain a clear photograph of yours. In case your Admit Card has been lost, obtain its copy from the official NTA website.
2. NEET UG 2026 Score Card / Mark Sheet
This is the official score card issued by NTA after NEET UG 2026 results are declared. It shows your Roll Number, Score, Percentile, and All India Rank. You need to upload this during registration and carry the original (or a clear print of the official NTA scorecard) at the time of college reporting.
3. Class 10th Marksheet (for Age Verification)
A copy of your 10th class marksheet is mandatory in order to prove your date of birth. If the date of birth is not indicated on your marksheet of 10th grade, then it is necessary for you to carry another birth certificate from the municipality authorities.
4. Class 12th Marksheet / Qualifying Exam Marksheet
This is the marksheet of your Higher Secondary (10+2) or equivalent qualifying examination showing individual subject marks for Physics, Chemistry, Biology/Biotechnology, and English. This is mandatory for verifying your eligibility for MBBS and BDS admission as per NMC/DCI prescribed minimum marks.
Important: If you appeared for Class 12th in 2026 and results are awaited, carry the provisional marksheet or school certificate confirming your pass status.
5. MP Domicile Certificate
This is perhaps one of the most crucial documents for MP state counselling. The domicile certificate should be:
- Issued by an authorized agency – SDM, Tehsildar or equivalent
- Valid and current – no expired domicile certificate will be entertained
- Issued in your name and not your parent’s name
During registration on the portal, you will need the Reference Number, Date of Issue, Place of Issue, and Designation of Issuing Officer from this certificate.
Note: Domicile is mandatory for Round 1. From Round 2 onwards, this requirement is relaxed if MP domicile candidates are not available. However, always carry your domicile certificate regardless of which round you are admitted in.
6. Category Certificate (SC / ST / OBC / EWS)
For those who come under the reserved categories, they need to have:
- SC/ST Certificate: Caste certificate which is permanent and has been issued to you by the concerned authority (SDM/Tehsildar or above). The caste certificate should bear your name.
- OBC Certificate: Caste certificate which has certified that you belong to a non-creamy layer OBC community in Madhya Pradesh. OBCs belonging to the creamy layer are not entitled to any OBC reservation benefits in MP counseling.
- EWS Certificate: Income & Assets certificate valid for the academic year 2026-27 issued by the concerned authority (Tehsildar or above). Note that EWS certificate is valid for a particular year, so you cannot present any certificate of an older year.
Important: EWS reservation is only applicable in Government Medical and Dental Colleges in MP not in private colleges.
7. Income Certificate of Family
An annual income certificate of your family for the financial year immediately preceding the year of the exam. This is required for fee concession assessment and also for EWS verification. It can be issued by a government officer or in some cases self-declared by the candidate as well check the latest portal instructions for acceptable formats.
8. Aadhar Card (Photocopy)
A photocopy of your Aadhar card is required as a supplementary identity proof. Note that for the “Photo ID Proof” field in the registration form, you are asked for a photo ID other than Aadhar (PAN Card, Passport, Driving Licence, etc.) the Aadhar copy is a separate requirement listed in Schedule 3.
9. Affidavit of Not Being Domicile of Another State
This is a self-declaration/affidavit stating that:
- You are not a domicile of any state other than Madhya Pradesh
- You have not availed and will not avail domicile benefits of any other state for admission purposes
A proforma for this affidavit is available on the DME MP portal. It needs to be executed on appropriate stamp paper and notarised in some cases check the portal for the current year’s proforma and instructions.
10. PWD (Disability) Certificate – If Applicable
If you are a Person with Disability (Divyang), you must carry:
- Disability Certificate issued by an authorised Disability Centre under Central or State Government
- The certificate must conform to the RPWD Act 2016, Section 58(3) provisions
- Previously, the Rehabilitation Centre in Jabalpur (under the Central Government) was the specified authority for issuing this certificate
11. NRI / OCI Related Documents – If Applicable
For NRI quota candidates in private colleges:
- OCI Card / Overseas Citizen of India document (if applicable)
- NRI Sponsoring document – proof that the NRI is sponsoring your admission
- Embassy-issued proof of NRI status of the sponsor (if not OCI)
- Affidavit in the prescribed proforma (available on the portal) regarding the NRI relationship and dependency
12. Freedom Fighter / Sainik Category Documents – If Applicable
Freedom Fighter Ward:
- Certificate issued by the concerned District Collector (District Magistrate) confirming that the candidate is the son/daughter, grandson/granddaughter, or great-grandson/great-granddaughter of a registered Freedom Fighter
Sainik (Defence) Ward:
- Certificate from the Unit Commandant (for serving soldiers) confirming active service in the Army/Navy/Air Force
- Certificate from the Zila Sainik Kalyan Board (for retired/deceased soldiers)
- Both sons/daughters and children of soldiers who died in service are eligible
13. MP Government School Student Certificate – If Applicable
This category was added through the May 2023 amendment to the MP Medical Education Rules. To avail the 5% reservation for Government School students:
- Certificate issued by the District Education Officer / District Coordinator / Assistant Commissioner confirming that the candidate has studied from Class 6 to 12th in an MP Government School and passed the qualifying exam; OR studied in government schools under Right to Education (RTE) from Class 1–8 and then continued in an MP Government School from Class 9–12th
This certificate must be presented in original.
14. Seat Leaving Bond (Seat Leaving Bond / Seat Leaving Declaration)
This is not a certificate you prepare in advance, it is a bond you sign and submit at the allotted college during reporting. It is a declaration that if you leave the seat after a certain point in counselling, you will pay the prescribed bond amount (Rs.30 lakh for government college; full course fee for private college).
The proforma for this bond is available on the DME MP portal. In some cases, it needs to be executed on stamp paper. Check the portal for the current year’s format.
15. Rural Service / Merit Student Bond
Also submitted at the college during reporting. This is a bond committing to rural/remote area service for 5 years after completing the MBBS programme (as applicable to government college admissions). Non-compliance attracts a bond amount of Rs.50 lakh.
16. MP Medical Council / MP Dental Council Registration Certificate
This is applicable for:
- Diploma/PG programmes – Candidates must have active registration with MP Medical Council or MP Dental Council (whichever applies)
- Not required for MBBS or BDS (undergraduate) fresh admissions
Important Instructions for Document Preparation
- All documents must be in original at the time of college reporting – photocopies alone are not accepted for verification
- Self-attested photocopies should also be carried in at least two sets
- Scanned PDFs for portal upload must be under 200 KB and clearly readable
- Do not upload blurry, cropped, or incomplete scans – verify using the “View” button after uploading
- Category certificates must be in your name – parent’s certificates are not accepted
- EWS certificates must be for the current academic year – check the validity date carefully

Q. Is an Aadhar card considered a domicile proof for MP NEET Counselling 2026?
A. Aadhar card is not an acceptable substitute for a MP Domicile certificate. You will require a domicile certificate from a qualified official like an SDM or Tehsildar in MP.
Q. The EWS certificate that I possess has been issued last year. Will it be valid for MP NEET Counselling 2026?
A. EWS certificates have different validity periods. For MP NEET Counselling 2026, you will require a valid EWS Income & Asset Certificate for the academic year 2026-27.
Q. Is it allowed to submit my scanned documents on the DME MP website in JPEG or PNG format?
A. Documents must be submitted in PDF format and less than 200 KB in size when submitting to the DME MP portal. You must first convert all documents/photos into PDF format before uploading. You can verify that your document was successfully uploaded by using the “View” button located on the enclosures page.
Q. I belong to OBC category but I am in the creamy layer. Can I still register?
A. You can register as a general/unreserved (UR) category candidate. However, you cannot avail OBC reservation benefits in MP NEET Counselling 2026.
Q. Is a disability certificate from a private hospital accepted for PWD category?
A. The disability certificate must be issued by a government-authorised Disability Centre as per the RPWD Act 2016, Section 58(3). Private hospital certificates are not accepted.
Q. Do I need to carry all documents to the college on the first day of reporting itself?
A. You must carry all original documents and self-attested photocopies on day one. If any document is missing, the committee issues a written notice. You will have a short window to submit the missing document but it is far better to be fully prepared on the first visit.
